Fight the Flu in Arkansas

In partnership with the Arkansas Department of Health (ADH), DeWitt School District is holding Flu Immunization Clinics to provide flu vaccine for students. For your child to receive the flu vaccine, parents/guardians must:

  1. Read the Vaccine Information Statement for the vaccine included in the 2017-18 ADH Consent Form links on this page.
  2. Read and complete the front and back of the 2017-18 ADH Consent Form.
  3. PRINT clearly all information required on the 2017-18 ADH Consent Form.
  4. Make sure you have signed the 2017-18 ADH Consent Form for the flu vaccine.
  5. Sign the 2017-18 District FERPA Consent Form link on this page.
  6. Return both consent forms to your child's school by the dates listed.

IMPORTANT! Consent forms MUST be returned to DES, DMS, and DHS by the dates listed. NO EXCEPTIONS. No forms will be accepted the day of immunizations. All forms must be received by date TBA, for verification by local health units.

This is a great opportunity for children to receive this vaccine at no charge to parents/guardians. If you have insurance, ADH will ask your insurance company to pay for the cost of giving the vaccine. If you do not have insurance or your insurance does not pay for vaccines, there will still be no charge to you.

REMEMBER, only those students with the required completed paperwork (the signed ADH Consent Form AND the DeWitt School District FERPA Consent Form) will be allowed to receive the flu vaccine.

Stay Healthy During Flu Season
Fluis a serious contagious disease that can lead to hospitalization and even death.  Flu-like symptoms include:  fever, cough, sore throat, runny or stuffy nose, body aches, headache, chills and fatigue.  Take everyday preventive actions to stop the spread the spread of germs.

  • Try to avoid close contact to sick people.
  • If you are sick with flu-like illness, CDC recommends that you stay home fore at least 24 hours after your fever is gone except to get medical care or for other necessities.  Your fever should be gone without the use of a fever-reducing medicine.
  • While sick, limit contact with others as much as possible to keep from infecting them.
  • Cover your nose and mouth with a tissue when you cough or sneeze.  Throw the tissue in the trash after you use it.
  • Wash your hands often with soap and water. If soap and water are not available, use an alcohol-based hand rub.
  • Avoid touching your eyes, nose, and mouth. Germs spread this way.
  • Clean and disinfect surfaces and objects that may be contiminated with germs like the flu.
